ABSTRACT

Objective:To investigate the effects of intravascular hypothermia combined with early post-pyloric feeding on the neurological function and prognosis in patients with severe ischemic stroke, and to provide a theoretical basis for clinical decision-making on the optimal nutritional support strategy for patients with severe ischemic stroke during intravascular hypothermia treatment.Methods:This was a retrospective, non-randomized, controlled study. A total of 78 patients with first severe ischemic stroke who were admitted to the ICU of Neurology Department, Xuanwu Hospital, Capital Medical University from January 2018 to December 2021 were selected. General information and clinical data of the patients were collected and grouped according to intrvascular hypothermia combined with nutritional support. Patients were divided into early post-pyloric feeding group of 52 cases and early parenteral nutrition group of 26 cases. The neurological prognosis, disease prognosis, nutritional status and complications related to nutritional support of the two groups were retrospectively analyzed.Results:The Glasgow score at 30th day after intravascular hypothermia in the early postpyloric feeding group was (11.25 ± 4.92) points, which was higher than that in the early parenteral nutrition group (8.40 ± 5.53), and the difference was statistically significant ( t=-2.45, P<0.05). After treatment, the serum total protein and hemoglobin of early postpyloric feeding group were (59.56 ± 5.09) g/L and (131.06 ± 19.58) g/L, respectively, which were higher than those of early parenteral nutrition group (56.52 ± 7.94) g/L and (122.07 ± 17.72) g/L. The difference was statistically significant ( t=-2.03, -1.91, P<0.05). The clinical pulmonary infection score of the early postpyloric feeding group was (7.33 ± 0.96) points, which was lower than that of the early parenteral nutrition group (9.42 ± 2.11). The mechanical ventilation time and ICU stay time were (17.46 ± 10.47) days and (28.89 ± 12.59) days, respectively. Compared with the early parenteral nutrition group (25.77 ± 15.20) days and (37.07 ± 17.15) days, the differences were statistically significant ( t=3.28, 2.83, 2.52, all P<0.05). There were no significant differences in catheter-associated bloodstream infection and ICU hospitalization mortality between the two groups (both P>0.05). Conclusions:Intravascular hypothermia combined with early post-pyloric feeding can improve the nutritional status of patients with severe ischemic stroke, effectively control pulmonary infection, shorten mechanical ventilation and hospital stay, and promote neurological repair.

ABSTRACT

【Objective】 To explore the rational management of contralateral patent processus vaginalis (CPPV) in laparoscopic high ligation of processus vaginalis. 【Methods】 A total of 300 children with unilateral oblique inguinal hernia/hydrocele who received laparoscopic high ligation of processus vaginalis in Baoding Children’s Hospital during Jun.2018 and Jun.2022 were selected and divided into two groups by random number table method, with 150 in either group. In the control group, 53 cases of CPPV were found intraoperatively, which were treated simultaneously. In the study group, 58 cases of CPPV were detected, among which 11 met the indications of high ligation and received simultaneous surgical treatment. The incidence of recurrence was compared between the two groups. 【Results】 After 1 year of follow-up, the recurrence rate was 8.62% in the study group and 1.88% in the control group (P>0.05). The detection rate of CPPV was 23.02% in children with unilateral inguinal hernia, significantly lower than that in children with unilateral hydrocele (49.07%, P<0.001). The detection rate of CPPV was 42.71% in children with left patent processus vaginalis and 32.95% in children with right patent processus vaginalis (P=0.19). The detection rate of CPPV was 62.93% in the age group of 1-2 years, significantly higher than that in other age groups (P<0.001). 【Conclusion】 The incidence of CPPV conversion into oblique inguinal hernia or hydrocele is low. Only children who meet the indications can be treated at the same time during surgery.

ABSTRACT

Objective To compare the pulp chamber temperature changes of Nd:YAP laser, Nd:YAG laser and semiconductor laser with the same power during dentin hypersensitivity treatment, and to evaluate the safety of these three laser treatments for dentin hypersensitivity. Methods 50 intact third molars were collected to prepare the dentin hypersensitivity model. The samples were randomly divided into Nd:YAP laser group (n=15), Nd:YAG laser group (n=15), semiconductor laser group (n=15), and blank control group (n=5). Each experimental group was divided into three subgroups (n=5) of 0.9 W, 1.4 W, and 1.8 W according to the laser power. The experiments were conducted with the corresponding laser parameters and thermocouple thermometer was used to record the temperature changes in the pulp chamber. The control group does not do any processing. After laser irradiation, one sample was randomly taken from each group and the morphology of dentin tubules was observed by scanning electron microscopy. Results When different power lasers were used to irradiate the samples, the temperatures of the pulp chamber in each group were increased. Among them, the temperature rise of the pulp chamber was smallest in the Nd:YAP laser group, followed by the semiconductor laser group, and the temperature rise was highest in the Nd:YAG laser group, but it was still lower than 5.5 ° C that could cause pulp necrosis. Scanning electron microscopy results showed that after irradiation with different power lasers, the diameters of most dentinal tubules in the Nd:YAG laser group and the semiconductor laser group were narrowed or even melted, and the effect was better than that of the Nd:YAP laser group. Conclusion The treatment using Nd:YAP laser, Nd:YAG laser and semiconductor laser for dentin hypersensitivity will increase the temperature of the pulp chamber. However, the temperature rise is less than 5.5℃and that will not cause irreversible damage to the pulp tissue. Nd:YAG laser and semiconductor laser have better dentinal tubule blocking effect, which is more suitable for laser dentin desensitization treatment than Nd:YAP laser.

ABSTRACT

Objective To study the application of photodynamic therapy (PDT) for dental caries prevention using whole body luminescence fiber,and to investigate the effects of PDT on the content of Ca and P in rat molar enamel.Method The rat dental caries model was established by inoculating with S.mutans.Eighty male rats were randomly divided into five groups,including three experimental groups:17 mW (8 mW/cm2) PDT (group A),34 mW (15 mW/cm2) PDT (group B),68 mW (30 mW/cm2) PDT (group C),a positive control group:20 g/L NaF solution (group D),and a negative control group:0.9% physiological saline (group E).The experimental groups were treated by 40 μg/mL hematoporphyrin monomethyl ether (HMME) and 650 nm diode laser irradiation.The experiments were conducted for 4 weeks.The contents of Ca and P in the molars of each group were measured by inductively coupled plasma emission spectrometry.Results The contents of Ca and P in group B,C and D after PDT were significantly higher than those in group A and E (all P<0.05).The contents of Ca and P in group A showed no significant difference before and after PDT,while those in groups B and C showed significant increase after PDT (all P<O.05).The increment of Ca in group A after PDT was lower than that in group D (P<0.05),while those in group B and C were significantly higher than those in group D (all P<0.05).There was no significant difference in the increment of Ca and P between group B and C after PDT.Conclusions In the range of the experimental parameters,the PDT promoted effect of tooth remineralization is better than 20 g/L NaF.The levels of Ca and P in the tooth enamel can be promoted by PDT treatment,and the contents of Ca and P are related to the pewer of PDT.The effect of low power PDT on the remineralization of enamel is not obvious.The contents of Ca and P in the tooth enamel are increased with laser power of PDT.When the laser power increased to a certain value,the change in contents of the two elements is not obvious.PDT can maintain the tooth remineralization microenvironment.

ABSTRACT

Objective This study used multivariate regression analysis to study the prevalence of malnutrition and the impact of malnutrition on independence in activities of daily living,health outcomes,chronic disease and healthcare cost among older Chinese adults.Methods This study was based on data from the 2013 China Health and Retirement Longitudinal Survey(CHARLS).The CHARLS data covers information on demographics,family,health status and functional limitations,health insurance,employment and earnings among mid-aged and older community-dwelling Chinese.This research focused on older Chinese adults aged 60 years and over.As a result,over 5,900 subjects were included.This study defined malnutrition based on body mass index,weight loss and reduced handgrip strength.Functional status was assessed via adapted versions of the Katz index of independence in basic activities of daily living (ADL)and the Lawton-Brody index of independence in instrumental activities of daily living (IADL).Health outcomes included self reported health status.Then multivariate regression analysis was used to investigate the relationship between malnutrition and various health outcomes.Results Thirty percent of older Chinese adults were malnourished.Malnutrition was associated with significantly lower IADL (OR =0.868,P < 0.05),poorer health status(OR =0.675,P<0.001),as well as higher probability of having stroke(OR =1.399,P<0.05)and chronic stomach disease(OR =1.241,P<0.001).As a significant contributor of poorer self-reported health status,malnutrition,in turn,increased hospital cost by ¥214 (14%increase,P<0.01)per person per year.Conclusions Malnutrition leads to functional limitations and worse health outcomes among community-dwelling older Chinese adults.These results imply that nutrition interventions such as nutrition screening and assessment,making scientific and personal nutrition support plans,and providing timely adequate nutrition support therapy can improve the health status and lower the healthcare cost of older adults living in the community.

ABSTRACT

Intravenous infusion and various injection Methods are the most common route of administration in the current clinical treatment. They play an important role in the prevention and treatment of diseases,but the complications are common in the clinic.In recent years,the concept of high quality nursing service has emphasized the prevention and reduction of the patient's discomfort, and significantly improved the nurses′understanding of the prevention and treatment of common complications of intravenous infusion and injection.In view of the commonly used method in clinical treatment of these complications include: 50% Magnesium Sulfate, 75% ethanol wet, cold compress treatment, local application of raw potato chips and local blocking etc.. The study found that the use of raw potato chips achieved good clinical effect on the method to relieve such complications.This paper from the raw potato chips for the recent progress in research on the acupuncture site complications of intravenous infusion extravasation,phlebitis and injection site in duration were summarized.

ABSTRACT

Objective To investigate the effects of Mycobacterium tuberculosis heparin-binding he-magglutinin (HBHA) on the polarization of mouse bone marrow-derived macrophages (BMDM) and a mu-rine macrophage cell line(Raw264.7 cells). Methods After HBHA was confirmed to be able to enter into macrophages by immunofluorescence, mouse BMDM and Raw264. 7 cells were treated with LPS (100 ng/ml)+IFN-γ (2.5 ng/ml),IL-4 (20 ng/ml),HBHA (1 μg/ml,3 μg/ml,6 μg/ml,10 μg/ml) and ESAT-6 (5 μg/ml),respectively. IL-6,IL-12 and TNF-α in the supernatants of cell culture were measured by ELISA. RT-PCR was performed to detect the expression of molecular markers of macrophage polarization [inducible nitric oxide synthase (iNOS), TNF-α, Arg-1 and CD206] at mRNA level. Western blot assay was used to detect the expression of iNOS and Arg-1 at protein level. Results Increased secretion of IL-6, IL-12 and TNF-α in the supernatants of cell culture and enhanced expression of iNOS and TNF-α at mRNA level were observed after treating BMDM with Mycobacterium tuberculosis HBHA. Similarly,in HBHA-trea-ted Raw264.7 cells,the secretion of IL-6 and the expression of TNF-α were also up-regulated. Mycobacteri-um tuberculosis HBHA and ESAT-6 had similar effects on murine macrophages. Neither of them could in-crease the expression of Arg-1,a molecular marker of M2 macrophages,in BMDM at protein level,but both enhanced the expression of iNOS,a molecular marker of M1 macrophages,in BMDM and Raw264.7 cells. Conclusion Mycobacterium tuberculosis HBHA can induce the polarization of murine macrophages to M1 phenotype.

In the era of outbreaking of antibiotic resistance, the discovery of new antibacterial drugs is emergent.Antimicrobial peptides are important components of the natural defenses of most living organisms against invading pathogens.The unique antibacterial mechanism, direct bactericidal effect, relatively slow of resistance acquirement, and can used alone or combined with antibiotics, make antimicrobial peptides be attractive potential antibacterial drugs.In this paper, we review the physicochemical property of antibacterial peptides, action mechanism and design of antimicrobial peptides, and give a brief introduction of several promising antimicrobial peptides.

OBJECTIVE@#To assess the effect of premolar extractions on third molar angulation changes in orthodontic patients.@*METHODS@#The Cochrane library, PubMed, Embase, China Science and Technology Periodical Database, China National Knowledge Infrastructure (CNKI), Chinese Biomedical Literature Database (CBM) and Wanfang database were searched from January 1, 1990 to May 20, 2014 to identify all the studies about third molar angulation changes in orthodontic patients with or without premolars extraction, which was assigned as a extraction group and a control group. Th e extraction group was further divided into a fi rst premolar extraction subgroup and a second premolar extraction subgroup. Literature filtering, data extraction and methodological quality evaluation were finished independently by two researchers. After cross checking, the disagreements were solved by discussion. Meta-analysis was carried out by RevMan 5.3.3 software.@*RESULTS@#Ten studies involving 712 patients were included. Meta-analysis revealed that: compared with the control group, the changes of third molar angulation in maxillary and mandible in the extraction group were statistically significantly different (all P0.05).@*CONCLUSION@#The orthodontic treatment involving first or second premolar extractions can improve the maxillary third molar angulation, and the second premolar extraction is the best option.

ABSTRACT

Objective We sought to evaluate the effect of structured health education on quality of life and the end point of patients with coronary disease with evidence-based method.Methods The Cochrane Library,Cochrane Database of Systematic Reviews (CDSR),Medline (OVID),Pubmed and Web of Science were searched to collect the randomized controlled trials (RCT) which mentioned the structured health education to CHD patients,the quality of the RCTs was evaluated by Jadad Score,then effective data were extracted and analyzed by Meta-analysis with RevMan 5.2 software.Results A total of 12 eligible studies of RCT were included in the study with Jadad Score evaluations at B level or above.There were 20 476 cases in the intervention group,14 522 cases in the control group.Meta-analysis showed that the quality of life scores of the intervention group were significantly higher than those of the control group,weighted mean difference was 3.29,95% CI (0.49-6.09).No significant difference was found in cardiac morbidity outcomes,re-hospitalization and the mortality rate,relative risk,95% confidence interval were respectively 0.72(0.50-1.04),0.98(0.86-1.11),0.77(0.58-1.03).Conclusions The structured health education can improve the quality of life of patients with coronary disease,but has no obvious effect on the cardiac morbidity outcomes,re-hospitalization and mortality rate.We suggest transitional care to be carried out in future research to decrease the incidence rates of them.

ABSTRACT

Objective To investigate the characteristics and clinical application value of anti-double stranded DNA antibody de-tected by Crithidia indirect immunofluorescence assay method and enzyme linked immunosorbent assay method.Methods Eighty-five patients with systemic lupus erythematosus,20 disease controls and 75 healthy controls were selected.The serum anti-double stranded DNA antibody was detected simultaneously by the methods of Crithidia indirect immunofluorescence assay and enzyme linked immunosorbent assay and their diagnostic efficacies for detection were compared.Results For each method the positive rate in the systemic lupus erythematosus group was significantly higher than that in the disease control group and healthy control group. The difference had statistical significance (P <0.05).The positive rates of Crithidia indirect immunofluorescence assay and enzyme linked immunosorbent assay in the systemic lupus erythematosus group were 72.94% and 88.24% respectively,and the positive predictive value of enzyme linked immunosorbent assay is lower(P <0.05).Meanwhile the anti-double stranded DNA antibody con-centrations detected by enzyme linked immunosorbent assay method showed statistically significant difference among the active sys-temic lupus erythematosus group,the stable systemic lupus erythematosus group and the control group (P <0.05 )and presented linear trend.Conclusion Using Crithidia indirect immunofluorescence assay method to detect anti-double stranded DNA antibody for the systemic lupus erythematosus group has high specificity and is helpful for the diagnosis of systemic lupus erythematosus. Enzyme linked immunosorbent assay can be used to detect anti-double stranded DNA antibody concentration quantitatively,which is linearly related with systemic lupus erythematosus activity and the method is of high sensitivity,which can effectively screen the pa-tients with systemic lupus erythematosus.

ABSTRACT

Objective Using the evidence-based medicine to investigate the effect of cognitive behavior intervention on physical and mental health in patients with type 2 diabetes mellitus.Methods Controlled studies related to psychological intervention on depression and glyeosylated hemoglobin in patients with type 2 diabetes were retrieved from electronic databases such as CNKI,VIP,Wanfang database and Pubmed,etc.The quality of included studies was evaluated and then data were analyzed by using RevMan5 software.Results A total of 273 articles were retrieved and finally 6 were enrolled.Results of Meta analysis were listed as follows:Cognitive behavior intervention could reduce score of Self-rated Depression Scale (SDS) in patients with diabetes mellitus.Cognitive behavior intervention could effectively reduce glycosylated hemoglobin values in patients with diabetes mellitus.Conclusions Cognitive behavior intervention can improve physical and mental health,decrease glycosylated hemoglobin and improve depression status in patients with type 2 diabetes mellitus.However,all the trials included in this review are of low quality; larger scale RCTs of higher quality are needed to confirm this conclusion.

ABSTRACT

Mannosylerythritol lipids (MELs), mainly produced by Ustilago and Pseudozyma, are surface active compounds that belong to the glycolipid class of biosurfactants. MELs have potential application in food, pharmaceutical and cosmetics industries due to their excellent surface activities and other peculiar bioactivities. In recent years, the research field of MELs has regained much attention abroad. However, MELs are rarely studied in China. In this review, the producing microorganisms and production conditions, diverse structures, biochemical properties, structure-function relationship and biosynthetic pathways of MELs are described. Some research problems and prospects are summarized and discussed as well.

ABSTRACT

Hospital-acquired pneumonia (HAP) is a very common complication in patients with neurological diseases.Its incidence and mortality are very high and result in a prolonged hospital stay and an increase in hospitalization costs.Therefore,how to effectively prevent HAP has become the focus of attention by neurologists.There are many risk factors for HAP,such as advanced age,conscious disorders,dysphagia,body position,oropharyngeal colonization,mechanical ventilation,enteral nutrition,and stress ulcer prevention drugs,etc.Clearing the risk factors for HAP and taking appropriate measures to strengthen protection may reduce the occurrence of HAP and improve the prognosis of patients.
